Be sure to complete your annual physical and the health assessment on Engage before Sept. 30, 2021 to qualify for the 2021 well-being incentive.
Confirm the claim for your annual physical has been processed by your insurance provider, and then check that Engage shows both requirements have been met.
Note: the claim for your annual physical must be processed correctly by your insurance provider and shown as completed on Engage no later than Oct. 31, 2021. Your health assessment must be completed by Sept. 30, 2021.
If you are in a Kaiser medical plan option, make sure you’ve also completed the Kaiser Wellness Program Agreement.
